Stevens Johnson Syndrome and Causes

SJS Symptoms

The condition often starts with vague symptoms of fever, sore throat, headache, joint pains, and gastroenteritis. These symptoms can last from 1 to 14 days. A widespread red rash then forms, which often (but not always) has typical spots known as “target” lesions. These are concentric rings of different shades of red, which may blister in the center, and they are often clearly seen on the palms. Similar blistering lesions then form in the mucous membranes such as the mouth, the airway, the gastrointestinal tract, the conjunctivae of the eye and the genital tract.

Potential symptoms of Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are as follows:

  • Rash, blisters, or red splotches on skin
  • Persistent fever
  • Blisters in mouth, eyes, ears, nose, and genital area
  • Swelling of eyelids and red eyes
  • Conjunctivitis
  • Flu-like symptoms
  • Recent history of having taken a prescription or over-the-counter medication

Stevens-Johnson syndrome (SJS) is a rare but serious disorder that affects the skin and mucous membranes. It is mostly caused by an adverse reaction to medications.

Progression of SJS

As these Stevens-Johnson related conditions progress, the skin may slough off. In Stevens-Johnson syndrome, epidermal detachment involves less than 10% of the total body skin area; transitional SJS-TEN is defined by an epidermal detachment between 10 and 30%; TEN is defined by a detachment greater than 30%.

Individuals of Asian Descent

In November 2008, the FDA warned that certain Asian ethnic groups carry a certain gene which causes an increased risk of Stevens-Johnson Syndrome. Scientific data has shown that Asians have a gene called the HLA-V*1502 allele, which, unfortunately, can cause an increased risk of Stevens-Johnson Syndrome from Dilantin. Before an individual is given Dilantin it’s extremely important to make sure to have a blood or genetic test to identify whether the gene, HLA-V*1502, which is a human leukocyte antigen allele is present in the body. This gene occurs in individuals of Asian ancestry, including people from China, Thailand, Taiwan, Malaysia, the Philippines and Indonesia.

On August 27, 2009, Intelence (Etravirine) the HIV medication made by Johnson & Johnson has been publicly acknowledged by Johnson & Johnson to have a relationship with severe skin conditions and reactions including Steven-Johnsons Syndrome (SJS) and Toxic Epidermal Necrolysis (TEN’s). For a drug to cause SJS or TENS means the drug could cause a systemic hypersensitivity reaction, oftentimes accompanied by liver failure. Intelence obtained FDA approval in 2008. Its approval is for the treatment of HIV victims who become resistant to other HIV medications. Intelence is a Non-Nucleoside Reverse Transcriptase Inhibitor.

The warning label on Intelence has now been strengthened concerning severe skin reactions. The new label states Intelence should be immediately discontinued if symptoms of severe skin or skin hypersensitivity develop.

Children’s Motrin & Children’s Advil

Children’s Motrin and Children’s Advil are over the counter children’s painkillers commonly given to relieve symptoms such as fever, body aches and pain associated with the common cold, flu and other childhood illnesses. Recommended for children ages 2-11 years, Children’s Motrin comes in liquid, chewable and tablet form and is recommended for children between the ages of 2 and 11. Children’s Motrin has been available in the non-prescription form since 1995.

There are many potential side effects associated with taking of Children’s Motrin, which include SJS, Toxic Epidermal Necrolysis, asthma, swelling, hives, and shock. Some of these side effects can cause severe reactions including blindness and death.

In December 2004, the parents of a 7-year-old girl filed a lawsuit against the maker of Children’s Motrin as well as several distributors alleging the painkiller caused their daughter to suffer serious side effects, including blindness and SJS. Doctors treating the child blamed the child’s loss of vision on Stevens-Johnson disease, which she allegedly developed the day after she took Children’s Motrin. The lawsuit also claims the pharmaceutical companies who manufacture Children’s Motrin knew about the associated risks but intentionally withheld that information from doctors and consumers.
